Issue of Duplicate Certificate

1. My original registration certificate issued by the Veterinary council of Uttar Pradesh has been lost/destroy, pertaining which a complaint has been lodged with the local police station and the copy of the FIR is enclosed in original.
2. I Would, Therefore, request you to kindly issue me a duplicate certificate.
